Advanced Practitioner Opportunity
St. Luke's Health System is seeking an Advanced Practitioner to join their highly collegial and patient focused Heart Failure Clinic, in Meridian. This role will be working day shifts Monday through Friday, no weekends. This collaborative team-based care model will be working with 4 APPs and 1 Primary Supervising Physician with additional support from a clinical pharmacist, dietician and social worker. You can expect day to day management of complex patients including initial heart failure work up, chronic disease management sometimes progressing to advanced heart failure treatment options vs end of life discussion. Work also includes collaboration with the Advanced Heart Failure Team from Intermountain Hospital. Cardiology (and/or internal medicine) experience is preferred for this position. Role Specifics & Qualifications: The schedule is M-F, no weekends or holidays. Shifts are typically four days per week, 10 hours.
