Provide HIV medical case management to program participants and assist in program activities and administrative support. Enroll clients into the program Provide intensive case management assessment and care coordination to clients Monitor participant's utilization of care with VCC and other agencies Coordinate appointments between clients and staff Qualifications
Minimum: High school graduate or equivalent Five years' case management experience or equivalent combination of education and experience Bilingual English/Spanish Valid California driver's license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record Availability and willingness to work evening and weekend hours, as required Required Skills/Knowledge/Abilities: Knowledge of co-occurring conditions Effective written/verbal communication skills Familiarity with local HIV/AIDS and non-HIV specific resources for health and social service needs Familiarity with health and social service needs of target populations served Basic computer knowledge, internet use, MS Office and data entry skills Pay Range: $22.00 - $24.00
