Keystone Human Services is currently seeking Supports Coordinators to join our team in providing individualized, person-directed support in the areas intellectual disabilities, mental health, and autism. Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer. As a Supports Coordinator, you will assist in the coordination of supports and services utilized by individuals with autism. This position organizes team meetings to develop and review Individual Service Plans, maintains documentation, schedules and coordinates services, and serves as a liaison between outside supports, individuals, families, and team members.
