Primary Care Group Physician Assistant Or Advanced Practice Nurse

Location: Harrisburg, IL Shift: Day Time Type: Full Time Facility/Clinic Name: Primary Care Group Position Summary Responsible for family medicine/primary care within the medical group under the supervision of an attending physician. Education Master's or Bachelor's degree in Physician Assistant Studies from a CAHEA accredited program Master's Degree in Advanced Practice Nursing Studies from a CCNEA accredited program. Licenses and Certification Current Illinois Licensed Physician Assistant or Advanced Practice Nurse Current NCCPA certificate as a Physician Assistant or AACC certificate as an Advanced Practice Nurse Active DEA license or ability to apply for such license prior to or upon hire Role Specific Responsibilities Assesses, diagnoses, and determines/alters treatment and management plans appropriate for age, acuity and clinical condition. This includes ordering, performing and interpreting appropriate diagnostic studies and prescription of pharmacologic and non-pharmacologic interventions and therapies in accordance with approved protocols/policies. Manages conditions based on clinical indication, evidence-based care, cost effectiveness and assessment of risks/benefits and alternatives. Provides care that is patient and family centered, compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of disease throughout the lifespan. Facilitates consistent, coordinated care and clear communication among all members of the healthcare team. Manages patients as part of an interdisciplinary team and within scope of practice. Formulates and implements outcomes-based treatment plans based on assessments, evidence-based medicine, standard professional practice guidelines and in collaboration with physicians and other interdisciplinary team members when appropriate. Performs procedures specific to scope of practice and knowledge of the provider as delegated by supervising Physician. Maintains accurate, complete, concise and timely documentation in the EHR. Substantiates and submits professional services consistent with compliant coding and billing practices. Act as a liaison between healthcare providers, the patient, family, and payers. Continuously build and maintain referral relationships with all medical staff. Assures the department/clinic meets professional and regulatory standards. Provides timely response to physician and patient correspondence. Completes education necessary to maintain licensure or certification required for position.
