Position Summary: Serves as an integral member of the assigned Trinity Health primary and specialty outpatient clinical care teams within the role of mid-level provider. Responsible for the provision of mental health services, case management support, staff consultation and training, and community education and outreach. Focus of duties will vary depending on academic training, experience and needs of the clinic/patients. What the Behavioral Resource Specialist will need: Masters in clinical social work or licensed professional counselors MA, LPC; MS, LPCEligible for licensure by the Michigan Board of Social Work as a Limited Licensed Master's Social Worker (LLMSW) or Licensed Master's Social Worker (LMSW). Completed post graduate supervision and full licensure with applicable experience in medical social work, counseling, case management. Preferred: One year of experience in medical social work, counseling, general social work. What the Behavioral Resource Specialist will do: Provides triage, comprehensive biopsychosocial and psychiatric assessment, psychotherapy, consultation and care coordination support to geriatric population in their homes. Provides individual, family and group therapy; short-term as well as traditional psychotherapy. Provide clients with self-management skills and educational information so they can be full participants in their own treatment and recovery. Monitors patient progress for achievement of health goals and outcomes. Maintains strong, working relationships with other members of the treatment team. Participates in case consultation and department meetings. Provides timely feedback to providers about the client's care, treatment recommendations, and progress via verbal communication and current EHR documentation. Provides consultation and training to Trinity Health providers and staff. Monitor and coordinate the delivery of health services including linking with other treatment providers as appropriate. LMSW is a key clinical resource responsible for delivering evidence-based behavioral health services to patients in an ambulatory setting. This role provides individual, family, and group therapy, clinical assessments, care coordination, and referrals. The specialist works closely with interdisciplinary teams to improve patient outcomes and ensure compassionate, culturally responsive care. Key Responsibilities: Deliver direct clinical services including psychotherapy, assessments, and care planning Utilize evidence-based interventions tailored to individual and family needs Coordinate care with primary care providers and interdisciplinary teams Document clinical notes, assessments, and progress in the EMR with accuracy and timeliness Assist patients and families in identifying and accessing community resources Provide culturally competent and trauma-informed care Engage in collaborative treatment planning and problem-solving May provide bilingual services if applicable Maintain compliance with all regulatory and ethical standards of clinical practice Participate in professional development and quality improvement initiatives Education & Licensure Requirements: Minimum: Master's degree from an accredited program in clinical social work Licensed by the Michigan Board of Social Work as a Limited Licensed Master's Social Worker (LLMSW) with a clinical designation Preferred: Fully licensed Licensed Master's Social Worker (LMSW) with clinical designation Certified Advanced Alcohol and Drug Counselor (CAADC) certification Experience Requirements: Minimum: Two years of experience in health care social work and/or counseling/psychotherapy Preferred: Two years of experience in an ambulatory care setting Knowledge, Skills, and Abilities: Minimum: Strong interpersonal and communication skills to support therapeutic relationships Proficiency with evidence-based treatment models at individual, family, and group levels Cultural competence and ability to work with diverse populations Strong analytical skills to assess needs and develop treatment plans Effective time management and multitasking abilities Strong written documentation and electronic health record skills High initiative and self-direction Collaboration and teamwork within clinical and support teams Preferred: Strong knowledge of local and regional community resources Bilingual (Spanish) capability Physical & Mental Requirements: Ability to manage high activity levels, distractions, and frequent interruptions Capacity for sitting, computer work, and phone communication for long periods Travel to meetings and across the facility as needed Ability to lift up to 35 lbs. and perform occasional physical tasks (bending, reaching, pushing/pulling) Fine motor skills and repetitive tasks required Must be flexible with hours and capable of troubleshooting diverse challenges Make a meaningful impact on your community while working in a supportive and collaborative environment.
