Bilingual RN Care Manager (Remote Flexible- Spanish Speaking)
Pair Team is hiring a full-time Bilingual RN Care Manager to support a high-need Medicaid population through proactive, whole-person care. This role is part of Pair Team's Care Management Program, which delivers integrated medical, behavioral, and social support to individuals with complex needs. The RN Care Manager will serve as the primary point of contact for a panel of high-risk members, contributing clinical expertise to build trust, address health disparities, and coordinate care across settings. This role plays a central part in an interdisciplinary care model, working alongside Community Health Workers, Behavioral Health Care Managers, and Nurse Practitioners to improve outcomes for individuals with chronic conditions, behavioral health needs, and/or significant social barriers to care. This position primarily allows for remote work; however, it includes regular in-person visits in the community across Clark County (Las Vegas). In-person activities typically occur 12 times per week, while the majority of duties are performed from home. Key Responsibilities:
Serve as the primary care manager for a panel of high-risk members, conducting regular outreach via phone, text, and in-person visits Conduct clinical assessments and develop individualized care plans in collaboration with members Provide health education, medication education, and chronic disease self-management coaching to improve health literacy and support behavior change Support transitions of care, including hospital discharge follow-up and coordination across providers and settings Perform medication reconciliation and help address prescription coordination, prior authorizations, or refills Identify clinical needs and provide brief interventions or triage support, escalating to internal clinicians when appropriate Monitor and close care gaps related to preventive screenings, chronic disease, and behavioral health Collaborate with the care teamincluding CHWs, behavioral health staff, and NPsto deliver whole-person care Track required assessments and screenings; ensure timely documentation of care activities Participate in regular case reviews and care team huddles to problem-solve complex cases and optimize care plans Participate in field-based member visits as needed to support engagement and care delivery What You'll Need: Active Registered Nurse (RN) license in the state of Nevada (required) Based in Clark County (Las Vegas) (required) Bilingual in English and Spanish (required) 2+ years of experience in care management, case management, or related RN-level community-based care roles Reliable access to a vehicle and willingness to conduct in-person visits 12 times per week Experience working with Medicaid populations and individuals with complex medical, behavioral health, and/or social needs Strong clinical judgment, communication skills, and cultural humility Ability to work independently in a remote-first environment while collaborating within a multidisciplinary team Proficiency with EHRs, documentation systems, and virtual communication platforms Valid driver's license and active auto insurance Because We Value You: Competitive base salary: $80,000 Comprehensive health, vision & dental insurance 401k Monthly work-from-home expense stipend Flexible vacation policy with paid time off Opportunity for rapid career progression and personal growth Work from the comfort of your own home Pair Team is an Equal Opportunity Employer.
