Licensed Practical Nurse (LPN) - Long-Term Care

Job Description: Job Title: Licensed Practical Nurse (LPN) - Long-Term Care Location: Waimea, HI 96796 Shift Schedule: 12-Hour Night Shifts (3 x 12-hour shifts per week) Duration: 13 Weeks (Possible extension depending on business needs) Guaranteed Hours: 36 hours/week Pay Range: $37-$40/hour per hour on a W2 Responsibilities: Deliver direct nursing care to residents in a long-term care (LTC) setting in accordance with care plans and physician instructions Administer medications and treatments while monitoring for side effects and effectiveness Conduct regular assessments and update documentation in compliance with facility and state requirements Collaborate with RNs, CNAs, physicians, and allied health professionals to implement individualized care Maintain accurate and timely records using electronic medical record (EMR) systems Respond to changes in residents' conditions, providing appropriate intervention and communication Follow infection control, HIPAA, and patient safety guidelines at all times Participate in care planning meetings, in-service training, and shift handovers Support residents' rights, dignity, and well-being in a compassionate, professional manner Skills/Experience Needed: Minimum 2 years of recent LTC (Long-Term Care) experience - Required Proficient in medication administration, wound care, catheter care, and vital sign monitoring Strong interpersonal, organizational, and communication skills Ability to work night shifts independently and collaboratively in a team environment Experience with electronic charting - Preferred Education: Graduate of an accredited Practical Nursing program - Required Associate Degree in Practical Nursing - Preferred Certifications: Active, unrestricted LPN License in the state of Hawaii - Required Basic Life Support (BLS - AHA) - Required COVID-19 Vaccination in accordance with facility policy - Required
