Job Summary
We are seeking a full-time 40-hour Diabetes Educator to support our Endocrinology department. The Diabetes Educator is responsible for providing comprehensive education and support to patients living with diabetes. This role focuses on promoting self-management skills to improve health outcomes while working collaboratively with healthcare teams to develop and implement individualized care plans and diabetes management programs. Essential Functions
Conduct thorough assessments to identify patients' educational needs and readiness for diabetes self-management learning. Develop and implement personalized education plans covering key aspects such as medication management, nutrition, physical activity, and blood glucose monitoring. Lead group education sessions focusing on diabetes management, prevention, and lifestyle modifications. Design and distribute educational materials and resources to enhance patient comprehension and engagement. Provide ongoing emotional support and counseling to patients and their families, addressing psychological challenges associated with diabetes. Qualifications
Education Associate's Degree Related Field of Study required Bachelor's Degree Related Field of Study preferred Master's Degree Related Field of Study preferred Licenses and Credentials Certified Diabetes Educator [CDE/CDCES] - Certification Board for Diabetes Care and Education required Experience Experience in health and nutrition related to diabetes education, counseling, or related role 2-3 years required Knowledge, Skills and Abilities Strong knowledge of diabetes pathophysiology, management strategies, and treatment options. Excellent communication and interpersonal skills to effectively educate and support patients and families. Ability to work collaboratively within a multidisciplinary healthcare team.
