Coordinates assigned program(s) to ensure clinical care is provided, consistently in assigned programs. In conjunction with medical leadership, responsible for creating policies, care pathways, standards of care and communicates with clinical/nursing staff working with the same cohort of patients. Responsible for monitoring volume, quality, care pathway compliance and reporting at appropriate venues. Supports the appropriate growth of programs and services by coordinating and supporting the development of protocols, clinical workflows, and education and research efforts of the team. Job Duties

Provides Direct Clinical Care to patients within assigned program. Focuses on assigned program and clinical population/service and serves as subject matter expert and resource across the program. In conjunction with medical leadership develops, implements, and monitors policies, clinical pathways, and patient outcomes. Develops standard protocols and workflows for patients needing to move through various departments as part of their treatment and ensures standards and time frames are maintained. Maintains functional knowledge of insurance, managed care plans, and medical authorization requirements. Utilizes evidence-based research, participates in research studies/projects, and communicates findings to co-workers and/or the professional community. Liaisons with the quality department to implement workflows and process improvement related to standard of care as they relate to regulatory, registry, and patient safety requirements. Implements protocols and workflows across the program. Develops monitoring mechanisms, across the enterprise and coordinates periodic meetings to present program status, inclusive of volume, outcomes, compliance with standards, and care pathways. Presents any challenges and solutions to achieve program goals. Serves as educator and trainer to clinical staff in practices, inpatient units, outpatient units, and surgical or procedural areas to ensure communication and consistency. Provides orientation, learning, competency, and care delivery training for the nursing and clinical support team. Collaborates with other program coordinators for common standards, and for appropriate transitions of care. Minimum Qualifications

Bachelor's Degree in Nursing 2 years demonstrated experience managing programs, clinical services, practices or other health care operations and 3 years providing care to patients within the specialty program Ability to think logically and sequentially, understand redesign concept, and identify opportunities for improvement. Ability to coordinate care of the patient across the care continuum. Ability to develop standards, training and communicate effectively. RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire Preferred Qualifications

Master's Degree in nursing or Master's Degree Certified Registered Nurse Practitioner (CRNP) 5 years in the care of patients whose needs match the program or 5 years in a management capacity overseeing delivery of care, with or without direct patient care activities CRNP - Certified Registered Nurse Practitioner - State of Pennsylvania Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
