Key Responsibilities: Ensure smooth daily operations at Case Management office Intensive HOPWA Housing Case Management to address barriers to maintaining permanent housing. Assess client eligibility for HOPWA services and prepare client intake/assessments to collect client demographic, income and asset information. Manage a case load range of 50-75 clients who are living with HIV/AIDS. Prepare individualized action plans identifying client needs and Perform primary Case Management responsibilities for HOPWA client families and short term housing assistance needs. Stabilize clients by providing the following eligible support services: housing information services, fair housing counseling, referral services and identification of resources to establish, coordinate, and obtain and/or maintain long-term housing stability. Coordinate, prepare and maintain required charting and documentation in a timely and thorough manner, including real time Provide Enterprise input and monthly reports. Adhere to all client confidentiality and HIPAA Coordinate, prepare billing and maintain monthly reporting for HOPWA Grantee. Adhere to all agency requirements and standards per HOPWA regulations. Reports to agency Executive Director.Develop relationships with community partners and representatives in other agencies to support individuals attaining services such as mental health care, psychiatric medication, medical resources, financial assistance, legal advocacy, Participates in department supervision, HOPWA trainings and other meetings and development opportunities as determined appropriate by supervisor. Qualifications: A Bachelor's degree in Social Work or related social science field from an accredited school is preferred or Bachelor's degree in another area. Prior supervisory experience required and/or experience in HOPWA housing case management. Experience using the Provide Enterprise system preferred. Bilingual candidates also desired. Effective verbal and written communication Strong and timely documentation and assessment skills. Strong team/consensus building skills. Ability to use holistic, client-centered approach. Ability to work with diverse population and demonstrate cultural sensitivity. Ability to effectively resolve conflict and cope with crisis situations. AHF Commitment
