Healthcare Staffing Professionals has an immediate need for a Senior Clinical Social Worker to support the Enhanced Care Management program. Enhanced Care Management (ECM) is a Medi-Cal managed care benefit that addresses clinical and non- clinical needs of high-need individuals through the coordination of services and comprehensive care management. Childrens Medical Services (CMS) administers the California Children Services, the Medical Therapy Program, the Child Welfare Public Health Nursing Program, and the CMS Edelman Childrens Court Pediatric Program. ECM-CMS program will serve this particularly vulnerable ECM populations of focus (POFs): Children/Youth Enrolled in California Childrens Services (CCS) with Additional Needs Beyond the CCS Condition. DPHs ECM-CMS team has extensive experience and expertise in working with this population for intensive care management and care coordination. It is anticipated that some CCS clients are also in the Child Welfare System. These clients, when encountered will be included in the initial launch. After the initial successful program launch, ECM-CMS will consider growth to fully serve both POFs. Intensive care management will be provided based on comprehensive care plans that aim at addressing the need for overall care management for the multiple systems with which the child/youth and family interact.
The positions will start immediately upon acceptance and employment clearance and are initially funded through 9/30/26 with potential extension.

Shifts:
On Site El Monte, CA Job Duties The HPM will work with the Senior Staff Analyst (SSA) on the development and implementation of policies and workflows to ensure a robust response in all of ECM's core service domains - outreach and engagement, comprehensive assessment and care, health promotion, comprehensive transitional care, enhanced coordination of care, individual and family/social support, and coordination of and referral to community and social support services. Coordinate and manage the social work services and ensure the delivery of high-quality, patient- centered care; Oversee the coordination of care for patients enrolled in the ECM program, ensuring timely access to appropriate services and resources. Coordinate ECM client assignment (externally with other ECM providers and internally across care management teams) for outreach and engagement; Create ongoing collaborative relationships with community and referral partners, and other public, governmental and client-centered organizations; Contribute to the development, implementation, and evaluation of ECM program policies and procedures. Skills/Experience:
Master's degree in Social Work from a graduate school accredited by the Council on Social Work Education inclining two years of clinical social work experience An active and valid license as Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences
A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
