APP Post Acute Care Opening in Manhattan, NY

Monday - Friday
17 patients per day
Perform primary care in a post-acute setting for short term and long-term patients
Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
Order and interpret lab results
Prescribe and monitor medications
Consult with physicians and coordinate patient care with other healthcare practitioners
Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
Promote healthy lifestyle changes
Compensation & Benefits
Compensation is fee for service (FFS): Expected compensation is estimated salary range of $111,000 to $161,900 annually with no cap on productivity income potential

Qualifications
A minimum of 1 year experience as a NP/PA in a primary care setting required
Post-acute care experience a plus, but not required
Active and unrestricted New York NP license required
Active and unrestricted DEA with prescriptive authority required
