Primary Care Advanced Practice Provider (APP) Opportunity

Scenic Northern New York Where the Adirondacks meet the St. Lawrence River

A well-established regional healthcare organization in northern New York is currently seeking a full-time Advanced Practice Provider (NP or PA) to join its primary care team at a modern, well-supported rural health center. This opportunity offers a collaborative environment, a flexible schedule, and a chance to make a lasting impact in a close-knit community setting.

Position Overview:
  • Setting: Outpatient primary care clinic located in Edwards, NY

  • Patient Volume: Approximately patients per day

  • Team Structure: Work closely with a supervising physician and a dedicated support staff including:

    • 2 Patient Service Representatives

    • 2 Licensed Practical Nurses (LPNs)

    • 1 Registered Nurse (RN) Care Coordinator

    • Office and Nurse Managers

  • Scope of Practice:

    • Conduct patient assessments and physical exams

    • Obtain comprehensive histories and create individualized treatment plans

    • Provide patient education on medications, health maintenance, and preventive care

  • Call Responsibility:

    • Rotating continuity of care call approximately one week every three months

  • Schedule:

    • Clinic hours are Monday through Friday, 6:30 AM 4:00 PM

    • Preferred schedule: Five 8-hour shifts with some flexibility within operating hours

This role is ideal for an experienced Advanced Practice Provider with a strong interest in community-based care and chronic disease management.

Compensation & Benefits:
  • Base Salary: $100,000 $200,000 (based on experience and qualifications)

  • Signing Bonus: $5,000

  • Relocation Assistance: Up to $7,500

  • Continuing Medical Education: $2,000 annually

  • Licensing and Professional Dues: $1,000 annually

  • Comprehensive benefits package including medical, dental, retirement plans, paid time off, and malpractice coverage
