A well-established regional healthcare organization in northern New York is currently seeking a full-time Advanced Practice Provider (NP or PA) to join its primary care team at a modern, well-supported rural health center. Position Overview:Setting: Outpatient primary care clinic located in Edwards, NY
Patient Volume: Approximately patients per day
Team Structure: Work closely with a supervising physician and a dedicated support staff including:
2 Patient Service Representatives
2 Licensed Practical Nurses (LPNs)
1 Registered Nurse (RN) Care Coordinator
Office and Nurse Managers
Scope of Practice:
Conduct patient assessments and physical exams
Obtain comprehensive histories and create individualized treatment plans
Provide patient education on medications, health maintenance, and preventive care
Call Responsibility:
Rotating continuity of care call approximately one week every three months
Schedule:
Clinic hours are Monday through Friday, 6:30 AM 4:00 PM
Preferred schedule: Five 8-hour shifts with some flexibility within operating hours
This role is ideal for an experienced Advanced Practice Provider with a strong interest in community-based care and chronic disease management.
Compensation & Benefits:Base Salary: $100,000 $200,000 (based on experience and qualifications)
Signing Bonus: $5,000
Relocation Assistance: Up to $7,500
Continuing Medical Education: $2,000 annually
Licensing and Professional Dues: $1,000 annually
