Hospice Nurse Practitioner Fargo, ND

A respected, mission-driven healthcare organization is growing its hospice team in the Fargo area and is seeking a Hospice Nurse Practitioner to join their collaborative interdisciplinary team.

Key Responsibilities:
  • Conduct face-to-face visits and comprehensive assessments to ensure hospice benefit eligibility

  • Provide symptom management and end-of-life care to patients in home settings, assisted living facilities, and skilled nursing facilities

  • Utilize telehealth to support rural or hard-to-reach patients as needed

  • Collaborate with physicians, nurses, social workers, and other team members to deliver high-quality hospice care

  • Educate patients, families, and staff on disease progression and treatment goals

  • Provide leadership in the application of advanced nursing practices

Territory & Schedule:
  • Average NP caseload: 5 patients/day

  • Hospice census across the organization: 400+

  • Schedule: Monday Friday, 8:00 AM 5:00 PM

Qualifications:
  • Licensed Nurse Practitioner (NP) in North Dakota and Minnesota (or eligible)

  • DEA licensure

  • Current driver s license and auto insurance

  • Hospice NP experience strongly preferred

  • Acceptable backgrounds include experience with geriatrics, palliative care, internal medicine, SNF, or related patient populations

  • Prior RN hospice experience a plus

Compensation & Benefits:
  • Salary range: $115,000 $140,000 (depending on experience

  • 27 days PTO (personal, vacation, sick, and holidays)

  • License fee reimbursement

  • DEA fee reimbursement

  • Up to 5 additional days off and $2,000 annually for continuing education
