We are seeking acompassionate and mission-driven Locums Primary Care Physician to join ourinterdisciplinary team in Fresno, CA as part of our Program of All-InclusiveCare for the Elderly (PACE). This unique, value-based care model enablesproviders to build deep, long-term relationships with a smaller panel ofpatients-focusing on quality, not quantity.

As a Primary Care Physicianwithin our holistic, team-based model, you'll experience the rewarding impactof working with frail, underserved older adults in a setting that prioritizescomprehensive, patient-centered care over volume-based medicine.

Compensation: $140-$160Per/hour+ Travel reimbursement.

Position Highlights:

* Provide primary care services to a panel of geriatric patients enrolled in the PACE program * Collaborate closely with an interdisciplinary team including nurses, therapists, social workers, and other specialists * Deliver patient-centered, preventive, and chronic care in an outpatient clinic setting * Daily patient volume is 8 in the clinic and 4 in field per day * Participate in care planning, case management, and care coordination for complex patients * Enjoy a manageable patient load and extended time with each participant

Qualifications:

* MD or DO degree required * Board Certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics * Active California medical license and DEA (or ability to obtain) * Must be eligible for enrollment with Medicare and Medi-Cal * Prior experience in geriatrics or working with frail, medically complex adults preferred
