Locums / Locum to Perm - Pensacola, FloridaGeriatrics Medicine - Medical Director Opportunity Position Details:

* Start Date: ASAP * Type: Locums or Locum-to-Perm * Schedule: Monday - Friday (Full-time) * Hours: Standard business hours * Call: Participates in provider on-call rotation
Practice Setting:
* Community-based model for patients 55+ with chronic care needs * Patients are certified to need nursing home care but live in the community * Services provided in a Program of All-Inclusive Care for the Elderly (PACE) setting * Only 7% of participants reside in nursing homes * Full continuum of medical and supportive services provided
Clinical & Administrative Responsibilities:
* Perform H&Ps on new enrollees; conduct reassessments as needed * Develop treatment plans and provide care across various care settings * Interview and evaluate prospective enrollees and caregivers * Participate in interdisciplinary team (IDT) meetings and family conferences * Collaborate with nursing team and supervise care plans * Supervise APPs and PCPs * Oversee QAPI program * Develop/maintain clinical protocols and standards * Educate patients, families, and clinical team * Order and interpret labs and diagnostics * Provide on-call coverage as scheduled
Leadership Role:
* Reports to Executive Director * Responsible for clinical outcomes, quality initiatives, and medical team leadership * Part of the executive leadership team for operational goals
Requirements:
* Board Certification in Geriatrics, Internal Medicine, or Family Medicine * Active Florida Medical License - Required * Strong interest in or experience with PACE models and community-based senior care * Prior leadership or medical director experience - Preferred
