Description Hello! Thank you for your interest in joining the Wellspring team as a Nurse Supervisor at our Covenant Skilled Nursing and Rehab at Wellspring. Covenant Skilled Nursing and Rehab at Wellspring is a partnership between Covenant HealthCare and Wellspring to help patients transition from the hospital to home. The new facility is part of a $5 million investment by Covenant and Wellspring to provide the most advanced post-acute care and services in the Great Lakes Bay Region. The Covenant Skilled Nursing and Rehab at Wellspring also provides physical, occupational and speech therapies performed by Mary Free Bed therapists in a newly renovated, state-of-the-art facility, working closely with the nursing department to assure the highest quality in post-acute programming.

As a Nurse Supervisor with the Covenant Skilled Nursing and Rehab at Wellspring, you’ll be part of a team of nurses, CNAs and senior support staff committed to helping seniors and families in Saginaw.

In this position, you'll be directly supporting your community in Saginaw County to provide services to seniors. In this position, you must be currently licensed by the State of Michigan Board of Nursing as a Registered Nurse (RN) OR as a Licensed Practical Nurse (LPN) successful completion of an accredited Nursing program. Two years of nursing experience with current home health, medical, surgical, or critical care experience is preferred.

As a Nurse Supervisor, you will:

Provide quality nursing care to residents in an environment which promotes their rights, dignity, and person centered care. Provide individualized attention, which encourages each resident's ability to maintain or attain the highest level of independence and physical, mental, and psychosocial well-being Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies such as lighting or equipment problems to the supervisor or appropriate staff Protect the personal belongings of each resident. Promptly report missing items according to established policy and participate in efforts to locate missing items Keeps nursing leadership up to date on resident status Communicate and interact effectively and tactfully with the resident, visitors, families, peers, and supervisors Perform all job responsibilities in accordance with prescribed safety and standard precautions Participate in meetings as required Assist with staff education, orientation, as requested Lead all staff on the unit during night shift/ other assigned shift, reporting to the DON/Nurse Manager on call as indicated Evaluate outcome of care and nursing judgments on a daily basis to assure high quality care Work with the MDS Coordinator in regards to ADL scoring Able to communicate with staff and monitor for any problems; follow up with DON if necessary and proceed with disciplinary steps as needed per policy Complete evaluations for nursing staff on the unit as directed Assure the aides are completing walking rounds and report to each other at shift change; assure that the nurse gives a thorough report to the aides; assure that the off-going nurse gives a thorough report to oncoming nurse Review EHR periodically to assure proper and timely documentation being done Assure documentation reflects the resident’s status, indicating progress, maintenance, or decline; must ensure that a resident’s abilities in activities of daily living do not diminish unless circumstances of the resident’s clinical condition demonstrate that decline was unavoidable Responsible to assure incident reports are filled out when needed, and properly done; assist DON with instruction when areas are incomplete or not accurately assessed Why Choose Wellspring?
