$24.00 - $36.00 Department Details In this impactful role, you will: Support multiple locations within 150 miles of your permanent address. Receive mileage reimbursement. Enjoy premium pay rates that include: $5/hour premium on all hours worked. $1.25/hour weekend shift differential. $1.25/hour evening/night shift differential (6:00 PM - 6:00 AM). Experience Requirements: A minimum of 3 months of Long Term Care experience (6 months preferred). Scheduling Requirements: Commit to at least 30 hours of work per month. Work at least one weekend shift per month. Job Summary As a Licensed Practical Nurse (LPN), you will provide professional nursing care for residents of all ages in long term care. Under the supervision of a registered nurse, advanced practice provider, or physician, you will: Demonstrate knowledge of nursing principles and deliver care according to individualized care plans. Administer medications or treatments as prescribed. Document all resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs. Work collaboratively within an interdisciplinary care team to ensure the health, comfort, and safety of our residents. Counsel residents and their families on prevention and treatment plans. Perform additional related duties as needed to support our mission. Qualifications Graduate from an accredited practical nursing program with a current unencumbered LPN license. Preference for an Associate degree in practical nursing. If IV therapy/management skills are required, completion of an approved IV therapy course is necessary. Must have had supervised clinical experience across the lifespan or a minimum of 400 hours as an LPN in another facility. Must be willing to complete required annual continuing education units (CEUs) and training.
