Therapist/MIHP Social Worker

The Corner Health Center is Michigan's first, largest and most comprehensive adolescent and young adult health center. The Corner provides high quality, judgment-free, affordable health and wellness care and education to youth and young adults ages 12 through 25 (and their children). Job Summary
The Corner Health Center seeks a Therapist/MIHP Social Worker. The person in this role will be responsible for providing confidential mental health counseling services to participants of The Corner Health Center (TCHC) programs and providing case management support to perinatal patients and their families. This is a non-exempt, full-time position with a competitive salary. This person will report directly to the Social Work Services Manager. This is an ideal position for someone with experience with diverse populations including LGBTQ youth and an interest in utilizing Evidence Based Practices.
Essential Duties and Responsibilities • Perform accurate assessments of their client needs to develop appropriate goals, progress, and treatment plans • Accurately submit fully completed billing and patient documentation in a timely manner according to company standards demonstrating services are medically necessary • Collaborate with the Corner Health Center team to connect families with additional resources or referrals as needed • Collaborate with organizational peers including nurse practitioners, physicians and therapists • Participate in trainings, seminars, and community education events • Provide care in accordance with all local, state, and federal laws and regulatory standards • Coordinate transition of care • Provide psychotherapeutic interventions appropriate to age, education, culture, ethnicity and functional ability • Provide therapy sessions to at least 13 non-MIHP clients per week • Serves as Social Worker on multi-disciplinary MIHP team (includes social work, nutrition and nursing) for on-site and at-home visits • Maintains consistent and thorough documentation, timely records and accurate billing • Functions as a vital member of the Social Work team • Provides intakes, discharges, and interventions as part of an interdisciplinary team • Participates in regular MIHP training • Commitment to the agency's mission and DEI initiatives • Provide services in accordance with the MDHHS CAHC Minimum Program Requirements and grant guidelines • Perform other duties as assigned
Qualifications: • Master's degree in social work from an accredited institution • Licensed to practice in the State of Michigan. Clinicians with limited licenses will be considered. • Infant mental health endorsement (IMH-E or IECMH-E) or a commitment to independently obtain endorsement within 6 months of hire is strongly desired. • Experience or ability to learn the Electronic Medical Record (EMR) Athena • Successfully pass background screening and not be in the OIG exclusion database • Excellent written and oral communication skills • Demonstrated ability to work with clients from diverse cultural, racial, and socioeconomic backgrounds. • Proficiency in Microsoft Excel, Word, Outlook, and PowerPoint • Commitment to the agency's mission and DEI initiatives • Provide services in accordance with the MDHHS CAHC Minimum Program Requirements and grant guidelines • Ability to work well as part of a team and independently
