Case Manager

You'll coordinate services and care for patients, providing coaching, strategies, and coordination of health and wellness resources so they can live their lives more fully. the clinic, patient homes and in the community. This is a great opportunity if you've just graduated or if you've had a little experience but you're ready for more. Essential Duties & Responsibilities Help patients navigate the health care system through coaching, education and support Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness Refers, advocates, and links client and family for additional services as identified. Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills Provides supportive and educational case management services to develop daily living and coping skills. Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family. Enters written documentation into Electronic Medical Record in a timely manner for all services rendered. Connect with clients face to face in the office and community, telehealth, and by phone to address needs Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners. Attend weekly staffing and team meetings and completes assessments Assist clients with developing natural support systems. Completes all required agency trainings to maintain 100% compliance. Maintains departmental productivity expectations. Attend and participate in clinical supervision. Knowledge, Skills & Abilities Qualifications Education Level Bachelor's Degree in psychology, social work or related field required Years of Experience Experience with Electronic Health Record preferred Certification/Licensure Valid driver's license in good standing required Physical Requirements Standing
