Senior Counselor/Health Coordinator

Under the supervision of the Assistant Program Director, the Senior Counselor/Health Coordinator is responsible for carrying a small caseload which usually consists of individuals that have co-occurring disorders. Provides crisis intervention, refers residents and appropriate levels of care. Assist the Assistant Program Director in monitoring residents when their primary counselor is not available. Responsibilities and Duties

The Senior Counselor is responsible for the completion of required documentation in case records for residents. Provide counseling 1X per week (30 minutes) Provides Crisis Intervention when needed. Facilitate support groups one time per week Refers resident to other levels of care Advocate on behalf of the resident when needed (i.e. Court Liaison, ACS/family court conferences). Responsible for maintaining up-to-date case records, filling out applications to be processed by specific agencies when needed. Assist in complying with Office of Addiction and Support Services (OASAS) and the New York City Department of Health and Mental Hygiene. Daily report to the Health Commerce System Ensures the provision of education, risk reduction, counseling, and referral services to all resident's regarding HIV. AIDS, TB. Hepatitis, STDs and COVID-19. Fosters abstinence and stability through counseling and other support Completes other assignments as requested by the Assistant Program Director Supervises the staff in the evening program when needed. Develop curriculums for Case Managers. Coordinate house functions for residents' chores Assist in maintaining the communication logbook. Network with out-patient treatment programs to ensure quality care for residents Qualifications

Associates Degree in human services or equivalent Must have CASAC Must be qualified health professional (QHP) with verifiable number At least 3 years experience as CASAC Must have valid New York State Drivers License Ability to work with diverse population.
