What Would You Do As A Forensic Discharge Planner - Grant Funded

In this vital role, you'll serve as the connection point between local jails, state hospitals, and Hampton-Newport News Community Services Board, working to ensure individuals living with serious mental illness (SMI) receive the support they need for a safe, coordinated discharge. You'll interview inmates, collect essential documentation, assess treatment needs, and determine eligibility for mental health referrals-all while crafting thoughtful, realistic discharge plans. As part of the Jail-Based Behavioral Health Service Team at the local jails, you'll help divert individuals from extended incarceration by identifying early intervention opportunities or facilitating rapid release when possible. Your role is key to reducing fragmentation in care and ensuring that individuals re-enter the community with access to critical services. Strong knowledge of state and local resources will enable you to build strategic linkages and contribute to a more compassionate justice system. To excel as a Full-Time Forensic Discharge Planner at Hampton-Newport News Community Services Board, candidates must possess a Bachelor's Degree in Human Services along with three years of direct care experience in case management and counseling services tailored to the client population. Proficiency in coordinating health care plans and interventions with multiple community agencies is essential. Familiarity with behavioral health consultation, screening, and case management functions dedicated to transitional care will provide a strong foundation for success in this role. Forensic experience is preferred, highlighting the need for candidates to understand the complexities of treating serious mental illness within the criminal justice system. Additionally, the position requires the ability to utilize relevant software and tools effectively. A valid Virginia Driver's License is necessary to facilitate travel between local jails and community service locations, ensuring comprehensive service linkage and coordination. QMHP certification is highly preferred to enhance your capability to serve this population effectively. Knowledge and skills required for the position are: Bachelor's Degree in Human Services and three (3) years of direct care experience in case management and counseling services to the client population assigned.
