Pulmonology Clinic (BHMG)
This is an employed position with the only hospital based multi-specialty group in the region. The practice is currently based in a large multi-specialty office with at least two exam rooms per provider and a full array of support staff. EPIC EMR is in place with proactive IT support. Outpatient office hours are 8:00am - 5:00PM and after hours and weekend coverage are provided by the inpatient pulmonology and critical care team. No Call! Clinic population - COPD, interstitial lung disease, pulmonary hypertension, lung nodules and lung cancer. We also co-manage connective tissue disease with either pulmonary hypertension or interstitial lung disease. It is a very diverse pulmonary population. Active interventional pulmonary program including navigation bronchoscopy, endobronchial ultrasound, tumor debridement, airway stenting, as well as bronchoscopic lung volume reduction, all done by our interventional pulmonologist. Sleep medicine department handles all of the sleep medicine referrals. If there is an interest in sleep medicine, some of the time could be spent in Sleep Medicine. Pulmonary hypertension meds other than continuous infusions are handled by our physicians. Right heart catheterization is done by our interventional cardiologists with vasodilator assessment if requested. Up to 16 patients on clinic days with up to 4 new patients and the rest are follow ups. They are booked as 30min or 60min slots. The current wait times for new patients is about 3 months. We receive about 25 new referrals per week. Outpatient routine bronchoscopy is supported by endoscopy staff using conscious sedation. Interventional pulmonology procedures are done in the OR with anesthesia support. We also have a full-time general surgeon that does surgical lung biopsy and a CT surgeon from AZ that comes up for more difficult cases. Minimally invasive lung surgery (VATS/Robotic) is done in Billings. We have a comprehensive lung nodule program managed by our interventional pulmonologist. This program follows all incidental lung nodules found on lung cancer screening scans both advising on appropriate management and making sure follow-up scans are completed. Full Pulmonary Function Testing is currently done through the hospital, with plans to move full PFT to the Pulmonary office in the future. We do not do cardiopulmonary stress testing.
