Integrated Behavioral Health Consultant
The Integrated Behavioral Health Consultant (LCSW/Clinical Psychologist) is embedded within primary and specialty care teams to work directly with the team to assess individuals with behavioral health and/or substance use issues and to provide brief treatment services within the primary/specialty care setting. The BHC is directly involved in the process of assessment, planning, implementation, facilitation, monitoring, and evaluation of the options and services required to meet an individual's health needs. They are patient advocates and counselors, perform psychosocial assessments as per SBIRT Program Objectives, and refer patients and families to Behavioral Health & Primary/Specialty care resources within the Einstein Healthcare Network. Through communication and identification of available resources, the BHC participates in the promotion of quality outcomes, patient satisfaction, cost-effective care delivery, and safe transition of care. The ideal candidate is knowledgeable about gender-nonconforming identities and expressions, and the assessment and treatment of gender dysphoria, including the evaluation of patients for Behavioral Intervention & Referral to treatment as appropriate. Essential Responsibilities: Interviews patients and gathers information relevant to the development of a plan of care for disposition. Documents patient screening questionnaire from SAMHSA and enters documentation into e-portal. Contacts behavioral health and social service agencies to gather information, after obtaining patient/legal guardian consent and connects the patient with these resources, either within or with external partners. Provides a summary of findings to attending provider and documents same in patient's medical record. Collaborates with attending provider and clinical team to develop a plan of care for disposition and documents consultation in medical record. Facilitates patient discharge/transfer to next most appropriate level of care. Qualifications: Education required: Master's degree in social work, Counseling, PhD/PsyD - Clinical Psychology or related social science field. License Required: LCSW, LPC, PsyD/PhD Clinical Psychology, equivalent. Experience required: Two/Three years in an Integrated healthcare/Primary care setting/behavioral health program or facility. Experience preferred: Care coordination, utilization/case management or discharge planning experience. Mental health/chemical dependence certification.
