Overview: We are seeking a dedicated Hospice Patient Care Supervisor, RN to join our compassionate team. In this vital role, you will report directly to the Executive Director, Administrator, or Senior Patient Care Manager, ensuring the highest standards of patient care within our hospice program. You will be responsible for managing patient care activities, guiding your team, and maintaining compliance with regulations.

  • Effectively oversee and manage visit requests, including rescheduled, declined, missed, and reassigned visits, using your clinical judgment to make priority and staffing decisions.
  • Respond promptly and compassionately to phone referrals and inquiries, triaging medical concerns within your scope of practice.
  • Prepare for, serve on, and facilitate the Interdisciplinary Group (IDG) meetings, presiding over discussions, presenting patient cases for review, coordinating meeting minutes, and soliciting input from care team members to adapt patient care plans accordingly.

About You:

  • Minimum of three years of direct patient care experience and two years in a hospice or home-based care setting.
  • Graduation from an accredited nursing school; a bachelor's degree in nursing is preferred.
  • Current Registered Nurse licensure in your state of residence.
  • Strong knowledge of hospice principles, industry standards, regulations (including Medicare, Medicaid, JCAHO, ACHC), as well as a deep understanding of the needs of terminally ill patients and their families.
  • Ability to provide appropriate advocacy and oversight by understanding the unique needs of hospice patients and families from diverse socioeconomic backgrounds.
