NP/PA Post Acute Care Opening in Syracuse, NY Seeking a NP/PA to work full-time (5 days per week) doing primary care rounding in a skilled nursing facility. Perform primary care in a post-acute setting for short term and long-term patients Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Order and interpret lab results Prescribe and monitor medications Consult with physicians and coordinate patient care with other healthcare practitioners Educate patients and their caregivers on coping skills to handle chronic and age-related conditions Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery Promote healthy lifestyle changesQualifications A minimum of 1 year experience as a NP/PA in a primary care setting required Post-acute care experience a plus, but not required Active and unrestricted New York NP or PA license required Active and unrestricted DEA with prescriptive authority requiredCompensation & Benefits Expected compensation is estimated salary range of $115,360 to $122,416 with no cap on productivity income potential.
