Responsibilities Position Summary:

Under the direction of the Pathways Program Manager, the Pathways Lead is responsible for enrolling at-risk community members into the Pathways program and providing comprehensive care coordination. This position will also assist Pathway Program Manager with assigned duties of the Pathways Community Health Advocates including training staff, documentation review, timecard review and approval. Assists in monitoring Community Health Advocates caseload, delegates referrals received from the Pathways Community HUB (PCH), reviews and provides input on Pathways-based care coordination plan, and signs off on all required CHA documentation within the timeline establlished by the PCH. Assists in assuring Community Health Advocaters are trained appropriately and provides agency specific training for new employees.

Qualifications Qualifications

Minimum:

High School Graduate General Studies or High School GED General Studies

Two (2) to four (4) years recent related Supervisor experience

General knowledge of medical terminology

Ability to communicate effectively, function independently, manage time efficiently, organize workflow and maintain confidentiality

General knowledge of health and social service resources that are available in the community to help participants achieve outcomes

Preferred:

Bachelors Degree in related field

Experience working with Community Health Advocates

Community Healthcare Advocate certification

Bilingual in English/Spanish

Computer skills: Google-docs, gmail, sheets, slides
