As a Clinical Care Manager, you will play a vital role in our Integrated Care Model by effectively communicating member needs, advocating for their care, and ensuring they access the right services tailored to their individual requirements. This position includes travel throughout various counties and allows for remote work flexibility if you reside in the state of New York. Primary Responsibilities: Assess, plan, and implement personalized care strategies directing patients toward the best and least restrictive options available. Conduct the NYS UAS Assessment in the member's home at least twice a year and as needed. Identify and initiate referrals for community support services, including financial, psychosocial, and state assistance programs. Manage care plans throughout the care continuum, serving as the consistent point of contact. Communicate critical health-related information to all stakeholders to ensure timely and coordinated care is provided to members. Advocate for patients and families to ensure their needs and choices are fully supported by the healthcare team. Required Qualifications: Current, unrestricted RN license in New York. 2+ years of relevant clinical experience. 1+ year of community case management experience with individuals who have complex needs. Experience in long-term care, home health, hospice, public health, or assisted living. Proficiency with MS Word, Excel, and Outlook. New York state-issued ID or the ability to obtain one prior to hire. Willingness to travel in assigned regions to meet Medicaid members in their homes or other environments, such as community centers, hospitals, or providers' offices. Preferred Qualifications: Behavioral health or clinical degree. Experience with electronic charting. Experience in coordinating community resources. Field-based work experience. Background managing populations with complex medical or behavioral needs. Proficient in the use of UASNY. The hourly rate for this position is $28.61 to $56.06, based on full-time employment.
