JOB SUMMARY:

Under the direct supervision of the CCO and the Recovery Services Manager provides a variety of relationship-based interventions including group counseling, crisis intervention, individual counseling sessions, staff coaching and training, admissions support and discharge planning oversight in addition to other relevant services to clients and company. The Case Manager is a clinical team leader providing comprehensive social, interpersonal, counseling and recovery services for the company and clients experiencing a range of SUD and MH concerns and life consequences.

MINIMUM QUALIFICATIONS

Education:

CADAC Certification I/II

Bachelors Degree- Prefered

Experience:

Addiction Counseling: 3 year reg.

Mental Health: 3 year reg.

License/CertificationRegistration:

BBS, CCAPP, CAADE, CADTP (Required)

ESSENTIAL DUTIES AND RESPONSIBILITIES

Under the direct supervision of the CCO and the Program Director, the Case Manager provides a variety of interventions including individual and group counseling, crisis intervention, and other relevant services to the clients. The Case Manager will work as part of an interdisciplinary team and provide services in a primary care setting.

Certification as a CAADE, CADTP, or CCAPP

Ability to create and maintain boundaries with clients and understand the limitations of a professional relationship within the work setting.

Ability to complete required program documentation on electronic medical records, KIPU.

Group facilitation ability.

Skill in managing multiple deadlines and demands.

Ability to follow through on assignments without intensive oversight.

Ability to work independently and as part of a group.

Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality and reporting laws.

Ability to work effectively with people of diverse cultural and educational backgrounds.

CORE DUTIES

• Motivational to encourage clients to follow their care plans

• Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records

• Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers

• Flexibility to change care plans if they are not getting the best results

• Organization to manage muiltiple cases at once

• Computer literacy to maintain and manage case records

• Assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans.

• Working with clients, family and support networks and health care professionals to put care plans in place

• Listening to clients’ concerns and providing counseling or intervention as required

• Recording clients’ progress including charting referrals, home visits and other notable interactions

• Evaluating clients’ progress and making adjustments as needed to improve outcomes

• Following up with discharged clients to ensure they were satisfied with services and they are still in good physical and mentalhealth

• Ability to manage high paced treatment center

• Able to organize and assist clients in obtaining long term planning, programming, housing, employment, etc.

• Demonstrated leadership skills able to initiate problem solving and program implementation.

• Working knowledge of diverse cultures and disperate income populations including the homeless and at risk.

• Ability to work flexible hours (including evening hours) to meet client needs

• Knowledge of HIPPA compliance to client/agency records according to established professional standards.

• Other duties as assigned, indicated or directed.
