Job Summary The Clinical Social Worker plays a vital role in providing comprehensive clinical social work and complex discharge planning for patients and families facing psychosocial challenges. This includes assisting with eligibility determination for social programs and funding sources, crisis intervention, and mental health assessments. Working within an interdisciplinary team, you will help ensure that the psychosocial needs of high-risk patient populations are addressed across their continuum of care. Minimum Qualifications Education:

Graduate of an accredited master's program in social work (MSW). Licenses/Certifications:

Current license as a Clinical Social Worker (LCSW) in Texas; ACM certification from ACMA is preferred. Experience / Knowledge / Skills: Three (3) years of healthcare social work experience is required. Acute inpatient hospital social work experience is preferred. Excellent oral and written communication skills are essential. Proficient understanding of DSM V and ICD-10. Demonstrated advanced skills in social work assessment, treatment of mental health status, and substance abuse screening. Strong therapeutic communication and negotiation skills with patients, families, and colleagues. Solid analytical abilities. Experience with utilization management, managed care, and payor issues is beneficial. Familiarity with ambulatory and post-acute care resources. Ability to work autonomously and collaboratively with a range of professionals. Principal Accountabilities Assess patients' and families' psychosocial vulnerabilities through comprehensive evaluations. Develop and implement effective treatment plans using clinical social work diagnoses and interventions. Address emotional, social, and financial ramifications of illness; mobilize community resources to meet needs. Provide short-term therapy, crisis intervention, and support in complex scenarios. Offer resources and counseling regarding treatment decisions and end-of-life matters. Advocate for patient and family autonomy in health care choices and access to services. Participate actively in discharge planning for complex patients to ensure successful transitions to post-discharge care. Facilitate communication and collaboration among multidisciplinary team members concerning discharge plans. Educate patients, families, and the clinical team about post-acute options and decisions. Supervise and mentor less experienced social workers and social work students in accordance with educational standards. Maintain adherence to Memorial Hermann policies and promote safe care for patients and staff. Engage in professional development to enhance skills and contribute to departmental and organizational goals. Demonstrate commitment to compassionate care and superior service experiences.
