The hospital's care team includes more than 2,300 nurses, technical professionals and support staff.Kent Hospital is an affiliate of the University of New England College of Osteopathic Medicine (UNECOM) for medical education.What You'll Do:Coordinate intake and pre-admission screeningsAssess patient suitability for admission through clinical judgmentEducate patients, families, and healthcare professionals on rehab options and facility expectationsServe as liaison with external case managers, discharge planners, and payorsConduct face-to-face educational sessions with physicians and case managersPerform market research and implement development strategiesGather and secure patient resources and benefit informationApply knowledge of developmental and aging processes to ensure appropriate care delivery Qualifications and requirements:Master's degree in social work,Marriage and Family Therapy or equivalent Professional Counseling pending State requirements.MSW, LCSW LPC-I, LPC, LMFT Independent Practice Scope Licensure in State of Practice; preferred.Minimum 1 year of experience in discipline; preferred.
