The High Fidelity Wraparound Family Support Partner will work as part of a Joint Planning Team to assist adolescents with emotional needs and their families. The Family Support Partner will provide education and limited direct support to help the adolescent and their family strengthen their natural support system. The Family Support Partner (FSP) position is designed to provide intensive levels of peer support for families. These positions are a distinctly different job than the HFW Facilitator, but they work closely with the Facilitator to support positive outcomes for the family. Job Summary

The High Fidelity Wraparound Family Support Partner will work as part of a Joint Planning Team to assist adolescents with emotional needs and their families. The Family Support Partner will provide education and limited direct support to help the adolescent and their family strengthen their natural support system. The Family Support Partner (FSP) position is designed to provide intensive levels of peer support for families. Essential Job Functions

Attend all mandatory county and state trainings. Attend meetings as requested by the adolescent, family and/or other team members. Support the implementation of the 10 Principles of High Fidelity Wraparound. Support the implementation of the 4 Phases of High Fidelity Wraparound. Must complete training in the High Fidelity Wraparound Model. Must work toward becoming credentialed as a Family Support Partner. Provide direct support (in a limited manner) within the family-driven plan. Ensure the Principles and Phase of the High Fidelity Wraparound process are delivered with the highest possible fidelity. Work closely with team members to support positive outcomes for the adolescent and family. Assist with linkage to natural and community supports. Assist with crisis stabilization when needed. Manage a caseload of up to 25 families Works with other team members and mental health providers as needed. Ensures completion of all documentation requirements within determined time-frames consistent with CGRC procedures. Other non-essential duties as specified by the immediate supervisor.

Required Education And Experience

Has had personal experience as a caregiver of children/adolescents involved with behavioral health services, the juvenile justice system, children’s protective services, special education, etc. Minimum age 18. Possess life experiences that allow them to form health working relationships with the adolescent and family as well as team members. Knowledge of community and natural supports which provide adolescents and families with opportunities to succeed. Ability to develop, manage and maintain relationships with others. Ability to remain understanding, patient, culturally aware and respectful to the adolescent, family, and additional team members. Ability to role model effective behaviors. Ability to support and improve self-efficacy and confidence. Ability to promote and strengthen health relationships. Ability to assist others working with the adolescent to understand adolescent “culture”. Possess strong communication skills and ability to utilize education to engage others. Possess excellent documentation skills. Flexible in adapting the weekly schedule to meet the needs of the adolescent and / or family. Possess a basic knowledge of daily living skills. Valid PA Driver’s License.

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, scanners and photocopiers.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is required to speak and hear sufficiently to communicate effectively by phone or in person at normal volumes. The employee’s vision must be adequate to read correspondence, computer screen, forms, etc. The position requires good manual dexterity to operate a keyboard, photocopier, telephone, and other office equipment. Employee is regularly required to sit, stand and walk, and lift and/or move up to 10 pounds.

Position Type/Expected Hours of Work

This is a part-time non-exempt position working 15 hours per week, Monday through Friday, including evenings and weekends as necessary.

Travel

Requires regular travel to other CGRC sites for meetings, trainings etc. and to outside community sites.
