Description

Zepf Center has been serving the Lucas County community for over 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.

General Summary:

To provide direct clinical and nursing services within both sites of Zepf Center, focusing on medical case management, with an emphasis on facilitating. To provide advocacy, linking, managing of psychiatric symptoms, evaluation of medication, medical case management, clinical and nursing service.

Hours: Monday - Friday; 8:00am - 4:00pm with some flex Tuesday evenings; 4:00pm - 8:00pm as needed

Essential Duties and Responsibilities: Provide clinical and nursing services as assigned to clients who have a severe mental health disability in the community mental health center setting. Obtain prior authorizations for medications and treatments for clients. Enroll clients in drug assistance programs. Medical case management for the medication only clients including treatment planning, monitoring of progress and completion of Ohio Outcomes. Follow-up on client no shows by contacting clients ascertaining needs and directing/coordinating with appropriate provider. Return client phone calls and triage client concerns, assuring timely communication with appropriate practitioner/provider. Obtain vital signs. Follow up on requested labs to facilitate communication with the doctor and the client. Assure medical record completeness for new clients and clients who are being discharged from the hospital and other settings. Review these charts to assure relevant information is available. Provide back up for the other nursing staff within the center. Perform other duties as assigned. Specific/Individual Competencies:

Requirements

Position Qualifications:

LPN Graduate from an accredited nursing school Three years experience in a psychiatric setting preferred Two years of medical experience preferred

Those eligible to drive company vehicles must have a valid driver's license and be eligible for coverage as defined by the agency commercial insurance carrier. Those who drive personal vehicles in the course of business must be able to provide proof of insurance.
