Job Description Salary: $25.00-$30.90/hour Job Title: Caregiver- Float FLSA: Non-Exempt Supervisor: Residential Administrator

Job Summary: Work as part of a team to provide comprehensive residential treatment and coordination of other medical and rehabilitative treatment for adults living in community based supported housing programs. Will work with an assigned caseload.

Schedule: On-call position where the schedule and location will vary. Our facilities are located in Greenwood, Northgate and two on Capitol Hill.

Essential Job Duties: 1. Ability to work non-traditional schedule as required for supporting clients at their housing site including holidays as assigned. 2. Provide immediate crisis intervention and resolution as needed. 3. When necessary, supervise residents taking medications, visually making sure they are taking correct doses at appropriate times. 4. Maintain resident and staff confidentiality at all times. 5. Have legible handwriting. 6. Be able to do simple mathematics. 7. Be able to read and comprehend agency/member charts.

Specific Duties Required of All Caregivers: 1. Provide assistance in Activities of Daily Living as needed for residents, per their service plan, and work with assigned caseload. Care includes assistance in hygiene needs, meal prep, laundry, and weekly assessment of living space for cleaning purposes. 2. Coordinate resident needs which may include but are not limited to assistance with medical appointments, outside agency services (PT, bathing, etc), therapy appointments or other appointments as assigned by supervisor. Accompany client to these appointments when assigned by supervisor. 3. Maintain member records in keeping with agency, county, state, and federal guidelines. 4. Provide input to residents primary care provider and/or case manager on resident progress and input on treatment plan. 5. Facilitate assigned group activity sessions and specialized resident events i.e. field trip, holiday party, etc. 6. Provide individual resident contacts minimum 1 time per week, when appropriate to resident emergency needs, per individual service plan, and as assigned by supervisor. 7. Assist in admission and discharge planning per supervisor direction. 8. Perform assigned residential activities which includes but is not limited to: a. Reads daily log and documents after each shift, informs other staff of any potential emergency situations i.e. maintenance problems, resident medical/mental status. b. Keep resident daily meal count when cook is not available 10. Attend scheduled staff meetings, community meetings, and when on duty and/or as assigned. 9. Monitor facility milieu for visitors, T.V. time, and quiet times. 10. Prepare resident meals when cook is not on duty and per menu. 11. Write progress notes in client(s) chart within 7 days maximum of each individual or group activity with residents. 12. Lead assigned life skills groups. 13. Coordinate fire drills, resident orientation, chores, and resident room maintenance during resident move-in and move-out. 14. Other duties as assigned.

Additional Duties of Caregiver Float: 1. Maintain and update Administrator checklist of client treatment plan and review dates, so Administrator can review and sign paperwork in a timely fashion. 2. Pass on necessary information from staff meetings to staff not in attendance and facilitate staff meetings when Administrator is not available. 3. In consultation with Administrator, set staff meeting agenda items. 4. In consultation with Administrator schedule, assign/perform intake interviews and ensure new intakes are staffed at the 1st available staff meeting. 5. Inform Administrator of any extraordinary client issues or staff performance issues. 6. Ensure weekly group activity schedule is set and posted. 7. Aide and inform Administrator of any shift coverage issues in a timely fashion. 8. Other duties as assigned per Administrator need.

Job Requirements: 1. Current First Aid and CPR cards. 2. Current Food Handlers Permit. 3. Current home phone/address. 4. Valid drivers license and clean driving record in order to use agency vehicles. 5. Documentation of personal insurance in order to use personal vehicle for member transport and to use agency vehicles. 6. 12 hours per year of continuing

education/training. 7. Tuberculosis test. 8. Agree to permit Criminal Conviction Inquiry. 9. Be free of substance abuse. 10. A working knowledge and adherence to all agency policies and procedures. 11. Certificate from DSHS 75 hour training within 120 days of employment and other specialized trainings per WAC.

Minimum Qualifications: 1. High School Diploma/GED 2. Experience with hands on care giving and/or ability to acquire specialized training to work with target population. 3.

Fundamentals of Care giving and ability to get other specialized training per WAC. 4. Ability to work independently and in a team.
