As a Thyme Care Palliative Care Nurse Navigator, you'll be a vital clinical resource for our members with serious illness and their care network conducting conversations about goals and values, advance care planning, and palliative care via phone, email, and video communication. Under the guidance of our Nurse Team Lead, you'll conduct comprehensive symptom assessments, support members with serious illness longitudinally, and facilitate end-of-life care discussions when appropriate. You will actively engage with members addressing clinical issues and efficiently managing any escalations that arise. The shifts for this position are either 8:30 - 5 EST or 11:30 - 8 EST. Additionally, we ask for flexibility with the occasional Saturday coverage. Adaptability: You are comfortable with change and ambiguity and have a proven track record of success in dynamic environments. Qualifications: A Bachelor of Science Degree in Nursing is required along with a compact unrestricted registered nurse (RN) license in addition to New York, Illinois, and/or Michigan licensure preferred. Additionally, you have at least 5 years of nursing experience including 3 years in outpatient palliative care and/or home hospice taking care of oncology patients and their caregivers. Certifications: It would be exceptional although not required that you are a Certified Hospice and Palliative Nurse (CHPN) or have oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM).
