Role: The RN, Case Coordinator, is responsible for evaluating and identifying patient/family needs, utilizing the nursing process, coordinating the Care Plan with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. Qualifications: Current RN license in the state where the employee will work Minimum one (1) year of experience for new nursing or hospice candidates; for internal applicants, a minimum of six months of LPN experience with hospice/home health care within an affiliate Previous experience working with an EMR/EHR system (Electronic Medical/Health Record) Valid drivers license and automobile insurance according to company policy Reliable transportation to meet visitation schedule Ability to use equipment with visual and auditory mechanisms Ability to communicate effectively in English (verbal and written) Flexible schedule to meet the needs of hospice patients Ability to handle the emotional stress of working with terminally ill patients and their families Ability to physically access patient rooms, other areas of the hospice/hospital/ALF, etc., and any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) within any part of the service area Ability to perform the essential functions and physical requirements of the job with or without reasonable accommodations (including, but not limited to: lifting patients and/or equipment, stooping, pushing/pulling, kneeling) Active BLS certification from the American Heart Association or Red Cross Some locations may require: Providing reassurance by phone to patients and their families. Helping to find solutions to their questions and/or recognize the need for an in-person visit. Coordinating the in-person visit when needed or requested. Helping the patient and their family to achieve desired comfort levels. Using appropriate resources or support/expert personnel to resolve complex or difficult situations. Documenting patient/family contact information in the EMR and communicating with the Interdisciplinary Group (IDG). Completing clinical admissions for all Company services, including, but not limited to:
Explaining services to potential patients/families and addressing patient fears, physical limitations, while reassuring the patient/family; presents services empathetically and compassionately Identifying admission objections that the potential patient or family members may have, concerns, or other barriers to entry and providing answers Educating the patient/family about admission requirements and addressing immediate needs Completing informed consents and initial assessment according to regulations and company policy Providing information to physicians and initiating a care plan to address the patients immediate needs Initiating specialized nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort Obtaining medical orders Completing documentation according to company policy
Acting as the Company representative in assigned facilities while facilitating referrals to all service lines; working closely with hospitals, physicians, centers, patients, families, and the general public. Communicating frequently with Admissions to ensure timely follow-up. When the task is completed, report to Admissions and prepare for the next task. Provide all necessary clinical communication on time using SBAR. Discuss any potential needs with staff designated to work outside of regular hours. Obtain consents for services and organize safe and efficient discharge of referred and eligible patients, while meeting productivity expectations. Obtain medical record information for the referral. Develop strong relationships with case managers, physicians, etc., in the facilities. Work with Professional Relations Representatives (PRR) to provide education about all available services. Provide care to any GIP patient in a hospital, including daily visits, professional management, documentation, and efficient discharge. Knowledge/Skills: Complete the requirements successfully to compete for this position. Responsibilities of All Employees: Represent the Company professionally at all times through the care provided and/or services rendered to all clients. Comply with all governmental, state, federal, and local regulations, maintaining a firm stance against fraud and abuse. Adhere to the Companys standard policies, procedures, and practices. Responsibilities of the Position: Provides and manages direct patient and family care as part of an Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical, and biological components, and appropriate nursing intervention and follow-up. Coordinates the Care Plan, ensuring that an individualized Care Plan is developed that accurately reflects the patients changing needs. Educates the patient, family, caregivers, and other health professionals about the disease process and deterioration, palliative interventions, care provision, the dying process, and safety practices. Reports changes in patient condition to the appropriate IDG members or other health professionals. Participates with the IDG to evaluate hospice referrals/admissions to determine the appropriate level of care. Attends patient deaths; prepares the body for transport; supervises the disposal of medications. Provides appropriate bereavement interventions to the family and caregiver in conjunction with psychosocial staff. Presents concise and pertinent oral and written reports to IDG; respects and encourages contributions from all disciplines. Communicates accurately and thoroughly with physicians, staff members, patients, families, and supervisors; uses positive approaches when working with others. Supervises patient care, provided by other members of the nursing team (i.e., LPNs, hospice aides, and contract nurses). Performs other tasks as assigned.
