Coordinate and oversee accurate, timely MDS assessments and care plans Collaborate with the interdisciplinary team to ensure the highest quality of resident care Ensure documentation is thorough, compliant, and reflects the true picture of resident needs Help track quality indicators and support QAPI initiatives Be the go-to for MDS guidelines, updates, and coordination What We're Looking For:
Current LVN license
in good standing (required) MDS experience preferred, or strong willingness to learn and grow in this role Strong understanding of RAI process and care planning Excellent communication, time management, and documentation skills A team-first attitude with a genuine passion for elder care Availability for a MondayFriday schedule Why Join Oak River Rehab?
