Pediatric Nurse Practitioner Primary Care

The Pediatric Nurse Practitioner performs the role utilizing the patient-centered coordinated care model demonstrated through the following competencies: leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making. Works collaboratively with other members of the practice/hospital team to assess, plan and implement care for individuals with health and safety needs. Monday thru Friday 8:30am-5pm. Practice locations Jackson and possibly Howell NJ. Responsibilities include providing medical assessments, treatments, clearances for HMH applicants, team members, constituents and external customers per specific company profiles & or regulatory standards. Performing clinical care professionally, serving as a customer advocate and role model. Clinical practice accurately reflects NJ Board of Medical Examiners regulations, Code of Ethics, the current NJ Department of Health requirements, and HMH policies and procedures. Developing and implementing appropriate plan of care. Facilitating patient teaching and support programs for patients/and employers. Evaluating current protocols and standards of care for translation of best practice into clinical practice. Supporting the development of evidenced-based practice protocols and standards of care that include interventions and treatments to achieve desired outcomes. Applying evidence and value-based practices to achieve positive outcomes for patients. Providing medication safely and effectively to promote effective recovery from on-the-job injury/illness. Prescribing medications and/or recommending over-the-counter medications as appropriate. Initiating laboratory tests, immunizations, and diagnostic test/procedures as appropriate according to regulation and scope of practice. Demonstrating competent clinical skills with treatment plans using most effective screening test or procedure. Collaborating with physician and consulting effectively with other members of the health care team. Demonstrating effective communication skills interdisciplinary to achieve results. Providing accurate, systematic and organized documentation of findings, recommendations/plan and education. Independently participating in continuing education and maintaining documentation. Developing a personal plan of development in area of clinical expertise, conferring with leader on goals and objectives. Providing culturally diverse and educationally appropriate education in relation to patient care/needs. Serving as a clinical resource to nursing staff, patients, and other multidisciplinary team members across the continuum to evaluate complex patients and identify appropriate interventions and evaluation of outcomes. Participating in policy/procedure reviews for updating with current evidence-based and value-based practices. Acting as a resource to staff for addressing clinical, process, and system opportunities for improvement which promote excellence in care and clinical outcomes. Promoting a collaborative work environment which supports the organizational mission, behavior expectations and values. Promoting innovation within clinical practice by seeking opportunities to bring new clinical knowledge into practice. Identifying ethical care issues and coordinating interdisciplinary collaborative meetings to resolve those issues. Floating to various clinic locations as needed. Other duties and/or projects as assigned. Adhering to HMH Organizational competencies and standards of behavior. Qualifications include a graduate of an NLN/AACN accredited program in nursing, a Masters Degree, maintaining clinical competencies through participating in continuing educational programs, and experience in relevant patient care setting or as regulations dictate. Preferred qualifications include a Master's Prepared Advanced Practice Nurse, minimum of 3 years professional experience, strong organizational and computer skills, effective verbal and written communication skills, emotional intelligence, critical thinking and effective interpersonal skills, ability to adapt to multiple and changing priorities, and a valid Drivers license. Required Licenses and Certifications include NJ State Professional Registered Nurse License, Advanced Practice Nurse Certification (w/ Masters Degree), and AHA Basic Health Care Life Support HCP Certification. Preferred Licenses and Certifications include CPR certification. Compensation starting at $117,395.20 annually. HMH is committed to pay equity and transparency for our team members. The posted rate of pay is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package. Posted hourly rates may be stated as an annual salary in the offer and posted annual salaries may be stated as an hourly rate in the offer, depending on the level and nature of the job duties and credentials of the candidate. The base compensation determined at the time of the offer may be different than the posted rate of pay based on a number of non-discriminatory factors, including but not limited to: Labor Market Data, Experience, Education and Certifications, Skills, Geographic Location, Internal Equity, Budget and Grant Funding. Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
