Community Living Case Lead (DSP/Supported and Independent Living, Full-Time or Part-Time) Toolworks'
Community Living Department
is seeking dedicated and compassionate
Direct Support Professionals (DSPs)
for
Case Lead roles
in both
Independent Living Services (ILS)
and
Supported Living Services (SLS)
programs. These roles offer a meaningful career path in supporting adults with disabilities to live as independently as possible in their communities. Location:
San Francisco Bay Area (San Francisco, Alameda, and San Mateo Counties) Compensation:
$24.18/hour Available positions and schedules:
Case Lead - ILS/SLS:
Full-time (Sunday-Thursday or Tuesday-Saturday) or Part-Time schedules available based on caseload and staff availability. Case Lead Floater - SLS Weekends & On-Call Support:
Friday at 3pm to Monday at 9am. May also receive assigned weekday shift hours for those seeking benefits eligibility. Position Summary
As a
Case Lead DSP
in our
Supported Living (SLS)
program, you'll provide in-home instruction, case management, and daily living skills training to individuals with developmental and physical disabilities. This includes support in areas such as: Meal preparation, personal hygiene, and budgeting Attending medical appointments and accessing community resources Travel training, shopping, safety awareness Supporting independence and inclusion in the community Each
DSP Case Lead
will work closely with an assigned caseload and help develop individual support plans tailored to each client's goals within the
SLS
and
ILS
service models. Case Lead Floater (SLS) - Key Responsibilities
Provide weekend and on-call support to clients and DSP staff Monitor the 24/7 Emergency Response System (ERS) and respond to staffing and client issues Cover open shifts during evenings, weekends, or overnight as needed Support SLS staff with training and guidance Earn a
$500 stipend
for full weekend rotation coverage or
$75/day
for partial coverage Client & Case Management Responsibilities (ILS/SLS)
Develop individualized service plans and support goals for clients in
Supported Living Assist clients in accessing benefits, building natural supports, and pursuing volunteer/work opportunities Foster self-sufficiency and community participation through DSP-led instruction and support Provide case notes, incident reports, and monthly billing documentation in compliance with
SLS
and agency standards Qualifications
Minimum 2 years' experience
as a
DSP
or related role supporting adults with disabilities in
community-based or Supported Living settings B.A. in Special Education, Rehabilitation Counseling, or related field preferred Fluency in
ASL , Spanish, or Cantonese highly desirable Strong communication, organization, and time management skills Supervisory experience preferred Flexibility to work mornings, evenings, weekends, or overnights Requirements
Fingerprint background clearance (DOJ, FBI, Child Abuse Index) Valid CPR/First Aid (can be completed within 30 days of hire) Clean DMV record and proof of auto insurance (if transporting clients) Fully vaccinated and boosted against COVID-19 (required unless exempt) Benefits (for eligible DSPs)
