  • Minimum Board Certification: Board Certified or Board Eligible (within 3 years of residency)
  • License Requirement: CA license required (open to providers willing to obtain)
  • CA Fluoroscopy Certification: Required or in progress
  • Providers will also need to have or being working towards obtaining the CA FLURO
  • The Following Certifications Are Required: ACLS
  • Estimated Credentialing Timeframe: 90 days
  • Fellowship: Required - Female Urology fellowship

  • Must supervise Advanced Practice Providers
  • No residents or teaching required
  • Facility will accept one provider only
