Job Description Overview CareConnectMD DCE is a specialized High Needs Direct Contracting Entity (DCE) geared towards Medicare beneficiaries who reside in a nursing home. Developed by geriatric professionals, the program provides a care model that is designed to meet the unique health care needs of nursing home residents. Under this value-based care model, CareConnectMD DCE will deliver care coordination services in close collaboration with primary care physicians, specialists, and advanced practice professionals in California, Georgia, Ohio, Indiana, Texas, and others on the horizon. www.careconnectmd.com Position Description The Project Manager (PM) works with project sponsors and teams to develop and track various projects, including tasks, responsibilities, due dates, outcomes reporting, meetings, outreach/training programs, and vendor coordination. The PM supports the High Needs Direct Contracting Entities (HN DCE) Program through process documentation, implementation, and tracking of key initiatives, coaching team members in an agile format. The PM is responsible for developing and tracking project plans, supporting meetings, developing agendas, taking minutes, following up, and tracking responsibilities. The PM will help maintain effective communication with team members, leadership, and the provider network. Key Duties and Responsibilities Coordinate and coach team members to maximize their contributions. Ensure understanding and adoption of the Scrum framework among employees and stakeholders. Act as a servant leader to guide the team in creating higher quality and higher value products by coaching on Scrum practices. Prepare and analyze data regarding outreach efforts, metrics, quality, and practice patterns; translate reports for varied audiences. Produce and edit deliverables such as presentations, letters, education materials, reports, and guidelines to keep stakeholders informed and address blocking issues. Prepare agendas, minutes, project summaries, and track project timelines for teams and leadership. Maintain files and documents related to project management and quality. Handle special projects assigned by senior management. Perform other duties as assigned. Education and Experience Bachelor's Degree in Business or a healthcare-related field; extensive work experience may substitute educational requirements at the employer's discretion. Minimum of 2 years' experience in project management. Industry Experience Hospital & Healthcare Information Technology and Services Research Licenses/Certifications Certified Scrum Master a plus Essential Skills and Abilities Excellent communication and interpersonal skills for all management levels. Ability to interact effectively with executive leaders, physicians, clinical and administrative teams, and office staff. Strong report-writing, statistical, and reporting skills. Proficient in PC software including word processing, spreadsheets, databases, and presentation tools. Ability to handle multiple tasks with short timelines, prioritize, and complete work accurately. Thrives in an unstructured, start-up environment. Self-starter with the ability to work independently and collaboratively, with initiative and enthusiasm for new tasks.
