About the Role As a Mental Health Clinician, you will play a vital role in coordinating evidence-based clinical services. You will collaborate with interdisciplinary teams to assess patient needs and develop tailored care plans, ensuring adherence to ethical standards and professional practices. Key Responsibilities Conduct comprehensive bio-psychosocial assessments and establish individualized care plans. Collaborate with team members to assess levels of care and risk factors, implementing and coordinating care and discharge plans to ensure optimal patient outcomes. Utilize therapeutic methods and interventions aligned with each patient’s unique needs, continually monitoring and adjusting their plans as required. Provide education and support to patients and their families regarding treatment goals, health maintenance, and disease management. Deliver direct patient care, including case management, crisis intervention, counseling, and referral planning. Maintain accurate and timely documentation that reflects assessment, interventions, and outcomes in line with regulatory requirements. Participate in team meetings, clinical supervision, and professional development activities. Contribute to programming development tailored to patient and community needs. Collaborate as a team member to enhance patient experiences while ensuring resources are utilized effectively. Qualifications Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or related field. Experience in a healthcare environment. Current licensure: LMSW, LPC-A, LMFT-A, or CADC. Valid Driver's License. Proficient in computer skills. Strong problem-solving abilities with timely decision-making capabilities. Excellent communication skills to effectively build relationships internally and externally. Cultural competence and sensitivity to health disparities, focusing on equitable care for all clients.
