Position Summary The Substance Use Treatment and Recovery Coordinator provides health education, resource and referral coordination, and linkages for GLIDE clients, particularly clients accessing overdose prevention and harm reduction safe use or who otherwise express an interest in treatment engagement. The Coordinator is responsible for conducting outreach and health education onsite especially where safe use supplies distribution occurs (i.e. GLIDE Service Hub, Eddy Street), providing proactive counseling support to clients, following up with clients to support treatment care coordination, progression, and adherence, and collaborating with internal GLIDE treatment staff (i.e. MOUD/MAT, Contingency Management, The Barber Shop, AA/NA, Congregational Life, Women’s Center, etc.) and external partners to support client access to treatment resources. This position is responsible for developing and maintaining an updated service and care coordination workflow and depository of resources for linking clients internally and externally to treatment options and health supports. This position requires direct client contact with diverse, high-risk individuals, many of whom are active substance users, and involves substantial community outreach. An active caseload of clients seeking and/or engaging in treatment services may apply. Regular and predictable attendance is required. Schedule includes 2 Friday evening shifts monthly at the Eddy St. SAS site distribution. Essential Duties & Responsibilities:

Coordinate the development, distribution, and education of substance use treatment health education, and resource materials, resource availability collateral in various accessible formats (e.g., language, text size, literacy) for community outreach and internal GLIDE staff training. Support internal stakeholders to understand in-house treatment coordination resources available and how to refer clients using established workflow. Update and outreach to external community partners to help them remain aware of ongoing substance use treatment resources at GLIDE. Provide 1:1 substance use treatment counseling and case management to an ongoing caseload of 10+ clients seeking treatment with GLIDE or other community partners (where appropriate) to achieve positive life outcomes. Provide clients with linkages, referrals, and problem-solving supports to address substance use, HIV/HCV/STI testing and treatment, primary medical care, behavioral health and wellness, benefits, and other social services. Provide proactive counseling, outreach, health education, and linkages to care alongside distribution of overdose prevention and harm reduction supplies, including safe sex and safe using supplies, to clients during operating hours at various sites (e.g. GLIDE Ellis St. site, Eddy St., etc). Provide client supports such as 1:1 counseling, coaching, and risk reduction education, and support group facilitation alongside/with other HEAT program activities where substance use treatment outreach and education are opportune. Coordinate incoming referrals from internal GLIDE programs or external community partners for substance use treatment options at GLIDE with the appropriate direct service staff and document efforts in Salesforce. Provide public group education and organizational presentations about evidence-based approaches for substance use treatment and public health best practices and intervention techniques for target populations. Complete, update, and document client intakes, assessments, care plans, progress notes, referrals, and linkages in required databases (e.g., Salesforce) to maintain accurate and timely client records for services provided in accordance with all state and federal privacy laws. Prepare and provide reports on a scheduled and ad-hoc basis for program development, funder reports, and evaluation efforts on substance use treatment coordination and program success. Participate in client and program evaluation activities for continuous quality improvement to ensure services are responsive and effective to achieve client outcomes. Adhere to professional boundaries and standards. Understands and follows basic ethics, values, and principles of case management regulatory practices. Adhere to all laws and requirements regarding HIPAA, confidentiality, child and adult protective services, contractual agreements, etc. Minimum Qualifications:

Bachelor’s degree in social work or related field OR 2+ years of substance use treatment case management, harm reduction counseling, and substance use treatment coordination for similar target populations. Experience working in “street-based” outreach situations and providing harm reduction services to people who use drugs, people experiencing homelessness, and/or people with co-occurring disorders preferred or equivalent experience in social services or health-related fields for a minimum of 2+ years Experience working with different substance use treatment modalities and community resources, and knowledge on how to navigate health/behavioral health care systems in the San Francisco Bay Area for people experiencing homelessness, poverty, living with infectious diseases, identifying as LGBTQIA, and BIPOC Knowledge of motivational interviewing techniques and methods for trauma-informed/harm reduction counseling and health education. Knowledge of effective de-escalation techniques for use in 1:1 interactions, during street outreach, and in group settings Strong interpersonal, oral, and written communication skills in order to present on a variety of issues to a range of stakeholders related to the role (e.g. client triage, service note documentation, outreach materials, email communication with referring community partners, etc.) Experience using MS Outlook Suite and databases for documenting program services and client progress (e.g. Salesforce, EHR) Basic math skills to track and calculate participation rates, program activities, and other service data points. Ability to plan and implement outreach objectives and activities, collect data and prepare reports; Ability to establish and maintain effective and professional working relationships with those encountered during the course of work, including clients, internal GLIDE stakeholders, external agencies, and the general public Ability to work independently and demonstrate ethical, objective, and sound decision-making in a fast-paced environment while maintaining a client-centered model Ability to maintain confidentiality and privacy of persons, documents, data, and communications, especially that which pertains to HIPAA; Demonstrates cultural humility and can work effectively across cultures. GLIDE’s buildings are located in the Tenderloin neighborhood. GLIDE maintains an "open door" policy to the community and its clients, who frequently enter and leave the building. Physical Requirements:

Ability to work on a computer and see details of objects at close range. Ability to hear within normal range, and communicate effectively (in person, telephone or Zoom). Finger dexterity and the ability to use all standard office equipment. Sit or stand comfortably, and the ability to navigate throughout office spaces (via elevator or stairs). $28.50 - $31.25 an hourThis is a full-time (40-hour/week) Non-Exempt position.
