The Opportunity

We're seeking an experienced healthcare operations leader with a strong clinical background to manage our comprehensive care team and serve as a critical bridge between our clinical operations and Advanced Primary Care (APC) services. This role is ideal for a seasoned pharmacist or registered nurse with management experience who can leverage their clinical expertise to lead a multidisciplinary team while ensuring seamless coordination with our primary care clinics. The successful candidate will be responsible for driving population health outcomes, managing clinical excellence, and extending our care capabilities beyond traditional clinical settings. Your Impact

Team Leadership & Clinical Oversight

Lead and manage a comprehensive care team including clinical pharmacists, registered nurses, LPNs, transitional care specialists, and care coordinators Ensure clinical aptitude and excellence across all team members through ongoing education, competency assessments, and performance management Develop and implement clinical protocols, care pathways, and best practices to standardize care delivery Foster a culture of continuous improvement and evidence-based practice within the team Conduct regular team meetings, case reviews, and clinical huddles to ensure coordinated care delivery

Strategic Partnership & Collaboration

Partner closely with the Medical Director of Advanced Primary Care to align comprehensive care services with clinical priorities and patient needs Collaborate with the leader of clinical operations to ensure seamless integration between comprehensive care services and clinic-based care Serve as the primary liaison between the comprehensive care team and primary care providers Participate in leadership meetings and contribute to strategic planning for care delivery expansion

Operational Excellence & Performance Management

Meet and exceed established caseload targets and weekly patient engagement expectations Monitor and analyze key performance indicators including patient satisfaction, clinical outcomes, and team productivity Develop and implement processes to optimize workflow efficiency and care coordination Ensure compliance with regulatory requirements, quality standards, and organizational policies Manage resource allocation and capacity planning to meet patient demand

Population Health & Outcomes Management

Drive population health outcomes through proactive care management and patient engagement strategies Analyze population health data to identify trends, gaps in care, and opportunities for intervention Develop and implement care management programs for high-risk patient populations Collaborate with clinical teams to develop risk stratification protocols and care plans Track and report on clinical quality measures and patient outcomes

Patient Experience & Care Coordination

Ensure the comprehensive care team delivers exceptional patient experiences that complement clinic-based care Oversee transitions of care between different care settings and providers Implement patient engagement strategies that promote adherence to treatment plans and self-management Develop and maintain relationships with community partners and external healthcare providers

Who You Are Education & Licensure:

Doctor of Pharmacy (PharmD) with current pharmacist license OR Bachelor's degree in Nursing with current RN license required. Master's degree preferred (MBA, MHA, MSN, or equivalent advanced degree). Relevant healthcare management certifications preferred.

Experience:

Minimum 5-7 years of healthcare management experience with direct reports. Clinical leadership & team development:

Proven experience leading and developing multidisciplinary clinical teams, fostering collaboration, accountability, and high performance. Care delivery & coordination:

Strong background in primary care, integrated care models, and population health, with expertise in care management, case management, medication management, and patient care coordination. Quality & performance improvement:

Experience leading or contributing to quality improvement initiatives and performance management efforts, with a focus on measurable outcomes and best practice implementation. Healthcare operations & regulatory knowledge:

Solid understanding of healthcare operations, regulatory requirements, and quality standards to ensure compliance and optimize care delivery. Technology & data-driven decision making:

Proficiency in healthcare IT systems and data analysis tools to support clinical decision-making and operational efficiency. Project & process management:

Demonstrated ability to lead projects, streamline processes, and implement system improvements in complex healthcare environments. Communication & stakeholder engagement:

Exceptional communication and interpersonal skills with the ability to build strong relationships across clinical and operational teams. Bonus Qualifications

Experience with value-based care models and population health management. Familiarity with care coordination software and electronic health records. Background in transitional care or chronic disease management. Experience working in integrated healthcare delivery systems. For PharmD candidates:

Clinical pharmacy experience, medication therapy management, or ambulatory care pharmacy background. For RN candidates:

Certification in case management (CCM), care coordination, or nursing leadership. Experience in quality improvement and patient safety initiatives. Why ConvenientMD?
