Summary:
As a Readmission Program Coordinator (Registered Nurse), you will play a key role in our mission to ensure exceptional healthcare by analyzing, recording trends, and developing strategies to reduce hospital readmissions. This role involves proactive data collection and analysis to identify causes of readmissions, suggesting alternative care approaches, and supporting care teams with evidence-based recommendations. You will also identify opportunities for improving care protocols to enhance patient outcomes and prevent readmissions. Reports to the Director of Quality. Performs other duties as assigned to support the quality department's mission.
Responsibilities:
Key Responsibilities:
Collect and analyze patient data, including readmission statistics, to identify trends, patterns, and risk factors to drive improvements.
Provide feedback to clinical staff based on data analysis to help refine patient care strategies.
Collaborate with hospital, UNC system, and community partners to improve safe and effective transitions from inpatient to outpatient care.
Input data into the Get with the Guidelines (GWTG) Database for heart failure patients.
Other Information:
Education:
Bachelor's degree in nursing required; master's degree preferred. Experience:
Minimum of 3 years of clinical experience, with at least 1 year in an acute care hospital or similar setting. Licensure/Registration/Certification:
Valid and current NC nursing license. Other Skills and Qualifications:
Experience in analyzing patient care trends, providing data-driven recommendations, strong analytical and problem-solving skills, and excellent communication and collaboration skills. Work Environment:
Schedule from 8:30 am to 5:00 pm, Monday to Friday, with holidays off. May require alternate schedules or additional hours.
