JOB SUMMARYCNAs and PCAs provide nonmedical service and assistance to a client in their home and community, who, because of advanced age or physical or mental disability, cannot perform these services. These services enable the client to remain in his/her residence. Therefore, the caregiver must feel comfortable caring for a client in a private home or setting. The Caregivers main objective is client satisfaction and safety for both oneself and for the client. The Caregiver must have sensitivity for the needs of others, a professional manner, and a positive attitude. Aides are responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with the Agency polices, industry standards, and state regulations. Aides needs to report on time to scheduled shift unless specific circumstances come up.ESSENTIAL FUNCTIONSThe CNA/PCA provides service to a client based on and limited to a client's care plan. The care plan is based on an assessment of the client's needs for services, developed in conjunction with a functional assessment and with the input from the client and/or client's family. CNA/PCA services are tasks that are necessitated by a client's disability or chronic condition that limits the client's ability to perform functional activities or daily living (ADL's) and instrumental activities of daily living (IADL's). CNA/PCA services vary depending on the needs and requirements of each individual client. Client needs and requirements are documented through the client's functional assessment. These services involve direct care from cueing/prompting to total assistance.Aides will provide excellent client care including, but not limited to, assistance with the following:AmbulationRange of MotionBathingPersonal HygieneDressingGroomingFeedingExerciseTransferringPositioningToileting and Incontinence CareMedication Reminders HousekeepingMeal Planning and PreparationLaundryTransportationShopping and ErrandsCompanionshipVital Signs and Blood PressureCNAs and PCAs will:Ensure client's safety and security by supervising the home environmentTeach/perform Meal Planning and Preparation and serving the clientRoutine Housekeeping activities such as making/changing bedding, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, and laundry. Household tasks should not exceed 20% of time worked per shift unless the client has a Housekeeping Authorization.Provide Companionship including social interactions, conversations, emotional reassurance, and encouragement of activities that simulate the mind.Provide respite care for families in accordance with care plansPreform/assist with essential shopping/errands, which may include handling the client's money in accordance with the care plan, agency policy, and under the observation of the Supervisor or Administrator*Assist clients with following written, special diet plan, and reinforcement of diet maintenance, which is provided under the direction of a Physician and as identified on the care planAssist clients with standing, walking, sitting, and rolling in bedEscort clients to medical facilities, errands, shopping, and outings as specified in the care plan*Assist clients with communication by writing or typing correspondence for them or researching information for themParticipate on the Care Team by providing input and making suggestionsNotifies agency when unable to work scheduled assignments-24 hour notification requiredEnsure service is delivered in accordance with all relevant policies, procedures, and practicesMonitor supplies and resourcesFollow the written care plan and works all hours as established by SupervisorCarry out duties as assigned by the SupervisorObserve clients and their environments and immediately reports unsafe conditions to SupervisorObserve clients and their environments and immediately reports behavior, physical and/or cognitive changes and/or changes in living arrangements to SupervisorComplete and maintain records of daily activities, observations, and direct hours of service.Submits written documentation to the office, as requiredAttend orientation, in-service training sessions, and staff meetingsDevelop and maintain constructive and cooperative working relationships with othersMake decisions and solve problemsCommunicate regularly with Supervisor and co-workersObserve, receive, and obtain information from relevant sourcesReport any incidents or accidents immediately to SupervisorConsistently follows Code of Conduct and policies as set for in the Employee HandbookMaintain client's protected health information confidential and observes all client's rightsReport, detect, and prevent abuse and neglectAides will perform other duties as required.
