Case Manager

Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, patient family, and other health team members while maintaining high standards of professional nursing. The Case Manager works as a part of the interdisciplinary care team, addressing the patient's quality and plan of care, goals and expected outcomes, and readmissions prevention/reduction strategies. The case manager coordinates discharge education while serving as a resource to the care team with progressive work towards discharge planning in both the inpatient and Emergency Department settings. What You Will Do: Accurate and timely record patient psychosocial assessments and interdisciplinary team meetings Recognize opportunities for documentation improvement and collaboration for plan of care. Effective and appropriate communication with providers, nursing and ancillary staff, and leaders. Ability to multi task and manage multiple priorities Accurate input of data and review of quality metrics. Demonstrate an understanding of medical complications, co-morbidities, severity of illness, risk of mortality, secondary diagnosis, and an ability to impart this knowledge to providers and other members of the interdisciplinary team. Evaluate nursing intervention outcomes, conferring with other health care team members as necessary. Gather and analyze information pertinent to documentation findings and outcomes. Timely follow up of culture results and communication. Understanding of social determinants of health screening and assisting the interdisciplinary team with communication of resources to the patient. Ability to manage patient's health concerns utilizing technology Regular and reliable attendance. Qualifications: Associate's Degree (AA) Graduate of an accredited school of nursing Required Bachelor's Degree in Arts/Sciences (BA/BS) Bachelor Degree in Science of Nursing (BSN) Preferred 1-3 years Acute Care Hospital Experience Preferred 1-3 years Prefer experience in Case Management and Discharge Planning Preferred Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures. Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions. Being aware of others' reactions and understanding why they react as they do. Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Adjusting actions in relation to others' actions. Talking to others to convey information effectively. Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. The ability to communicate information and ideas in speaking so others will understand. The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position. The ability to arrange things or actions in a certain order or pattern according to a specific rule or set of rules (e.g., patterns of numbers, letters, words, pictures, mathematical operations).
