Home Health Nurse
Responsible for nursing care of assigned patients. Carries out physician directed nursing services in the home setting requiring the application of the biological, physical and social sciences and nursing skills in the care of the sick, the prevention of disease and the conservation of health. Reports directly to the Home Health Clinical Supervisor Carries out physician directed nursing services in the home setting. Cares for patients of all ages who are admitted to Home Health. May be assigned as Admissions Nurse. Follow-up with payor sources for authorization/reimbursement purposes. Coordination of care between disciplines. Participates in RN on call rotation as assigned. Maintains good organizational skills, efficient time management and timely paperwork submission. Minimum of one year clinical experience in acute medical/surgical care (hospital, SNF, or LTAC) required. Home Health experience preferred. Current Louisiana RN license or multi-state compact license. CPR certification. Possess and maintain valid drivers license. Maintain required hours of continuing education. Ability to communicate with patient, family, peers and medical personnel; Ability to exercise independent judgment. Requires emotional stability and maturity; Must be adaptable to change, be decisive, and demonstrate foresight; Exhibits organizational and leadership ability. Work requires a variety of physical activities including standing and walking for long periods of time, bending, stooping, and lifting. The work environment may be physically confining. Visual acuity is required for a variety of specific activities. Must have normal hearing or utilize adaptive hearing devices if duties require listening with a stethoscope to discern breath sounds, heart sounds, and bowel sounds.
