Sub - Care Manager - Harm Reduction and Low Barrier Services

Provide case management and specialized intervention services to clients in the syringe exchange and low barrier buprenorphine program. This includes assessment, service coordination, risk reduction knowledge and skill development, treatment planning, crisis intervention, HIV counseling/testing/referral, HCV counseling/testing/referral, drug overdose prevention, and related services.

Sub - Care Managers cover open shifts due to vacancies and planned/unplanned time as they have availability.

Responsibilities:

Prepare and distribute harm reduction supplies Provide education to the population on safe use and harm reduction and provide training and assistance with overdose reversal tools (eg. Narcan) Help clients navigate complex systems of housing, healthcare, recovery, and other social work organizations Provide emotional support to clients in crisis, support emergency medical staff in the case of overdoses or injuries, and participate in creating a culture of safety and non-judgement Requirements:

A combination of education and relevant experience Valid driver's License and use of a personal vehicle Structure:

Hours vary Non-Exempt Pay rate is $23.45/hr Benefits:
