Registered Nurse
to join our collaborative and patient-centered team in a unique hybrid position. This role is structured as a
3-day work week :
two days will be dedicated to direct patient care , and
one day will be focused on phone triage
support. This position offers a rewarding opportunity to blend hands-on patient interaction with system-level care coordination and triage support, while maintaining work-life balance with a condensed weekly schedule. The
Triage Registered Nurse
will support and help further develop a regional patient phone triage system within Praxis Health Southern Willamette Valley. In addition to triage responsibilities, the RN will provide direct patient care two days per week at Oak Street Medical Clinic. The ideal candidate is both clinically proficient and patient-centered, with a strong understanding of care coordination and communication across multiple settings. Direct Patient Care (2 Days per Week):
Deliver high-quality, compassionate nursing care in a clinic setting.
Perform routine clinical tasks such as assessments, immunizations, patient education, and care coordination.
Assist providers with procedures and exams as needed.
Maintain accurate and timely documentation in the electronic health record (EHR).
Foster a supportive environment for patients, families, and fellow staff.
Triage Nurse Duties (1 Day per Week):
Monitor and respond to triage calls received via task, the nurse phone line, and the patient portal.
Using established protocols: Communicate directly with patients via phone to assess symptoms and offer home care advice or appropriate next steps.
Determine urgency of care needs based on protocols and review of patient history.
Direct patients with high-risk symptoms (e.g., chest pain, abdominal pain) to the ER or coordinate emergency response.
Schedule same-day or next-available appointments for non-emergent care needs.
Consult with clinicians when further guidance is needed beyond protocol.
Assist in developing standard messaging for common patient inquiries.
Serve as a clinical resource and professional role model for the clinical team.
Additional Responsibilities:
Participate in the development, refinement, and support of the regional triage system.
Collaborate with leadership and providers to enhance triage processes and ensure regulatory compliance.
Help develop policies, procedures, and workflows relevant to the triage role.
Engage in regular staff meetings, trainings, and planning sessions.
Communicate professionally with staff, patients, and community stakeholders.
Strive to reduce conflict and improve patient satisfaction through timely, empathetic communication.
Perform other duties as assigned.
Minimum Qualifications
RN License Minimum two years in direct patient care, preferably in outpatient care setting. Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills. Competent computer skills and knowledge of HER systems. Excellent problem-solving skills. Innovative, forward-thinking mindset. Demands high work from self and others. Demonstrated ability to work effectively with diverse races, ethnicities, ages, and sexual orientation in a multicultural environment. About Us

While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to sit and stoop, kneel, crouch. The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds. Work Environment: Combination of medical office and exam/procedure room settings. OSHA Category This position performs tasks that involve exposure to blood, body fluids and/or tissues.
